All this "landing at the wrong airport" will never happen if pilots stick to instrument approach and follow all the procedures.

Haven't heard about it. In KUL we do mostly ILS approach, visual approach is very rare even for the locals due to the number of traffic around. Therefore to mistakenly doing approach to the new runway 33 is highly unlikely.
